TB CLEARANCE (PPD or MANTOUX) Must be done within 1 year of child starting school in Hawaii. TB must also show Date TB test was Given, Date TB test was Read, Results of the TB test in MMs (pos or neg is not acceptable), & a dr's/nurse's signature/stamp.

PHYSICAL EXAMINATION must be done within 1 year of child starting school in Hawaii and must have a Dr's signature. All immunizations must be current according to age.

Original BIRTH CERTIFICATE (we will make a copy and return your original back to you).

PROOF OF RESIDENCY. Must be Utility Bill (ex. Telephone (not cellular), Cable, HECO, Water) or Lease or Rental Agreement. Residency must show LEGAL PARENT/GUARDIAN NAME. Bill must be current.

According to state law, Hawaii Revised Statutes (HRS) §302A-1143 and Hawaii Administrative Rule (HAR) §8-13, school-aged children are required to attend the school in the area in which they (legal parent/guardian) reside.
Parent/guardians need to be informed that, according to HRS§710-1063, falsification on a government application is a misdemeanor and that when such a violation is found, the child will return to the school where he/she should properly be attending.
If the parent or legal guardian cannot provide documentation of legal residence because the parent/legal guardian is living with a relative/friend, a notarized statement by the relative/friend can be accepted by the school with the following stipulation:

Notarized statement must state that the parent/legal guardian and child are living with the relative/friend;

Notarized statement must state the name of relative/friend that is confirmed by the relative/friend’s proof of legal residence;

Notarized statement must state the same address of relative/friend that is confirmed by the relative/friend’s proof of legal residence;

A copy of the relative/friend’s proof of legal residence must be attached to the notarized statement; and,
-
Notarized statement must be signed by same name of relative/friend that is confirmed by the relative/friend’s proof of legal residence.
STUDENTS TRANSFERRING FROM ANOTHER PUBLIC SCHOOL

If student is transferring from another public school, parent must submit the following:

Release packet from the previous public school

Completed Student Enrollment Form, MVA Form, and Student Internet Use Form

Current Proof of Residency
