The School Health Aide (SHA) provides the first aid care for injury & illness. The SHA's responsibilities include: 1) First aid - to protect the life & comfort of the individual until additional care is secured if necessary, 2) Monitoring & maintenance of students' school health requirements, and 3) Administration of daily/emergency medication.
No medication will be stored in the Health Room or administered by the authorized Public Health Nursing Branch and/or DOE personnel without the completion of form PHN/SH36, Rev. 5/12, & other supporting forms. The SH36 form is available at the following website: http://www.hawaii.gov/health/family-child-health/publichealthnursing/index.html. This form is good for the current school year and needs to be renewed yearly.
Medication must be in a container/vial dispensed by the Pharmacist with instructions "FOR SCHOOL USE" with the name of the student, name of the medication, dosage, strength, time of administration, and name of prescribing physician.
